Add Alert Text Field to Billing Rules

Add an "Alert Text" field to the billing rule definitions. If a procedure code has alert text associated, then the text would be displayed to the user whenever the code is added to a bill for the first time. An example usage would be to warn the user that another procedure code is required for the bill when a procedure code is used that can be accompanied with a tray fee procedure.
