Predefined Chronic Conditions Templates

Have the ability to define a chronic condition template so that when a patient is assigned a condition in the EMR>Chronic Conditions, data will automatically populate as defined by the clinic/physician. As it stands now, most of the data in this section has to be chosen each time a condition is attributed to a patient, when you have many thousands of patients this is a alot of work especially if patients have multiple conditions. Having the ability to map to this section so that when a patient is given a chronic condition a 'base' amount of data is brought in. If patient A has condition Z, if specific labs / documents / medications etc exist in the encounter notes section as defined by the mapping of the practice, they would automatically populate into chronic conditions and allow the end user to have a base as well as add more if they require.
