Add another column in gear boxes to allow query forms to be seen

This would be great for billing staff to see that a query has been created in all areas of view. We have a 6 month window and its the only way to keep track of old queries as there is no report to reflect them. It would be better to have that visual everywhere. We don 't adjust to pay these claims as they just disappear when you do. We can't be right clicking on every claim to find them or be viewing the billing history to follow a claim.
