Ability to Fax a form to a referring doctor in one step

I created a notice of appointment form and set it as a chart sheet, so the secretaries can create an appointment, and hit F9 to bring up the form fully populated with patient name, appointment time, etc. The form includes the referring physician, and referring physician's fax number, but this info is not getting passed through when they fax the form. Instead, they have to open the address book and select the referring doctor to fax. Would be much better if the fax number got passed through automatically, as it does with a template.
