This is great, but now you have to make this functionality work for Forms. So many forms request certain parts of medical history, and the only current medical history widget in the forms is not editable and is in an atrocious format (long and skinny, but most forms go sideways)

This can be accomplished using the Template Wizard's "Query" function.
Tools > Template Wizard > Select or create your template > Put your cursor in the body of your template where you would like the information to display > Click on the Query icon (blue speech balloon) > Medical History > Set your parameters > <OK>
This will create a "tag" the same as right-clicking in the template body and selecting from the menu, but the parameters are more robust, allowing you to specify formatting, and whether to include associated dates and details (including medication dosages).
