Patient's next appointment in forms

Patient's will have three appointments of different types booked for them at one time. Would like to create a form with the appointment time and date for all three of these appointments queried onto the form. Currently the only options are to choose the Patient's Appointment date and time, and the Patient's next appointment date and time. Require the ability to choose more than just the 'next' appointment and the current appointment. Ideally, if we could use something such as "Patient's next appointment with Type ____" So that we could choose multiple 'next appointments' But the date and time is going to be different based on the Type of appointment that was booked. Or even just options such as Patients next appointment, Patients second next appointment, Patients third next appointment, etc.
