Form Description - Add component

When you open a new form in patients chart there is an option at top of the form to add a description. The data in this field is used in multiple locations, most important for our clinic in Outstanding Reqs/Refs/Orders. Would like to be able to add a component to the form directly that does the same job as this description field with the ability to require a verifier would be awesome ! We find that the description field gets missed by the end user alot and having the ability to place this directly on the form with its own/different heading will encourage usage.
