Improved "Create Bill" for query results

The "Create Bill" tool in the query results window is a useful tool, but it is a blunt instrument. You can do batch claims on the resulting list, but you have to specify a single date and single provider. This makes no sense. Since you've just done a query it is most likely that you have a mixed list of patients, so what we really need is to create bills with extra options: Date: select a date column from the results list. Provider: add option "enrolled provider" add option "office provider" select a provider column from the results list The above would make the query+create_bill pathway very much more powerful. We also need other options besides "Create Bill", but I'll create a new item for that.
