Additional Address on forms

There are times when a patient has two addresses. One is a mailing address (ex a box number) and the other is the actual house address. We put the mailing address in the main address area and the house address by putting it in the additional address space. However, on some forms, such as the one for the Mobile Lab, they require the physical address. Currently I am unable to put this address on the form. I see we can label an address as residence, mailing, or business. If we could specify that the "residence" address be placed on the form, that might work in this instance.
