Change form editor tags to a more logical listing order: ie name address city location postal code

In "form editor", the listings under patient, family physician, etc are not listed in logical sequence. Having a FAX number tag wedged between city, location or Postal Code is absurd. Every tag listing should be re-edited to place them in logical order in each category. This is so obvious, it should never have been allowed to happen in the first place!
