Ability to pull immunization summary into a form

It appears all other medical history items are available to be queried in a form except Immunizations. Can we please add this functionality.

    Commenting on this again as it would be a great feature to have (also it includes dates when each vaccine dose was received).


    Would also be great to have dates in this code for macros/notes: <MHXIMMUNIZATIONSUMMARY>.
