Appointment Creation Date Component

It would be helpful to have a component that would pull the appointment creation date onto forms within the form editor. This would be helpful to keep track of appointments, when they were created and show the original appointment date if it had later been changed. This would also show the duration of how long the patient has been in contact with the clinic.
