Ability to Select Group Appointment Date of Service in Forms

Customers would like the the ability to choose the appointment date of service in a form when opened on any of the patients included in the group appointment. Currently you have the ability to select a regular appointments DOS or -- none-- from the dropdown in Forms, but clinic's would like the ability to select the Group Appointment if they are completing a form regarding that Group Appointment.

    It would also be great to have it available in the templates section.
