Using Custom Forms to Automate Processes

I have two processes that I would like to automate using a custom form. 1. Patient Diagnosis Assistant This form uses a series of categorized check boxes, Example Diabetes T2, Diabetes T1, Hypoglycemia. A Dr can select one or many check boxes for a given PTs diagnoses. Each check box will add a new CPP diagnosis with description and ICD9 associated code. WHO: all physicians WHY: it provides a streamlined solution for consistent data entry of PT diagnosis HOW: Dr would open the form, choose the diagnosis required and submit. The form would populate the CPP with all diagnosis details and diagnosis code DIFF: currently Dr must follow the manual process for adding a diagnosis and code to a patient record. Its time consuming and it relies on the Dr to use the correct nomenclature and diagnosis code. This process creates inconsistent data. Additionally, it would be nice to have a form the could populate other EMR variables such as billing and assigning tasks right from the form or launching additional forms. To further automate this process. 2. Encounter Tracking for MOH reporting I would like to have a custom form that records all the variables for mandatory MOH reporting. There is a great example of such a solution that I am using in one of my Practice Solutions locations. This automated from is designed with conditional fields so that depending on choices the form either expands with additional data elements or populates other variables based on choice. A visual example would work much better in describing this form. WHO: all IHPs WHY: it provides a streamlined solution for consistent data entry encounter data, it provides an consistent reporting method for MOH reports, it ensures consistent data entry HOW: IHP opens form, completes all the encounter details fore each PT, external reporting database is used to report on the extracted data from the EMR DIFF: currently all IHPs are using home grown adhoc methods of recording the PT encounter statistics for MOH reporting. paper, counting appointment history, excel spreadsheet, guessing are all currently used to obtain this data.
