medication review and best possible medication history

We need the ability to document the 'best possible medication history', i.e., what the patient is actually taking of the medications prescribed (both active and external medications). We need to be able to document who reviewed the patient's medications with them, and when, as well as to document any differences between what has been ordered/prescribed and what the patient is actually taking. Adding fields to the medication tab to accommodate this aspect of medication management might include leveraging the existing review functionality (green check mark) for encounter notes--reviewed/not; by whom; on what date/at what time. The existing comments field might be useful to document any pertinent details, if it could be modified by anyone other than the prescriber. This would need to be applied to both active and external medications. RNs and other health professionals are often involved in medication review and cannot document in the medication tab; documenting this informaiton in the encounter note is not helpful to those generating new prescriptions or refilling existing ones.
