Under history of problems in the encounter notes tab, have the default go to the description, not the CODE

When you're entering a condition or diagnosis under history of problems, it would be helpful to have the cursor default to the Description rather than the Code field.  We have hundreds of codes we use, and there would be no way to memorize them by code number.  It would make more sense just to default to the description, if possible.
