Take the "Mrs" default for all females off.

It offends single women to be called Mrs.and it is not appropriate for children. How can we correct the patient's that already have the incorrect title without updating them one by one.

    The User Preference for default title will only apply to any new patient's created moving forward. There isn't a way to retroactively change the title on all patients in mass.

    You can change your default for when entering new patients.  Under File, User Preferences, Defaults Gender and Title.
