User name in booked appts

For an appointment, please add the user name of the user who booked the appt in the apt details so that we can see who booked the patient. This would be useful for scheduling accountability, and everyone in the office including the doctors would benefit from this.

Since you can view the username for each action (Created, edited, moved, etc) on each scheduled appointment in the F4 window, Quick Patient Appointments View.

This request is to also show that information right in the Appointment Details window.

You can see who booked each appointment and made changes to the appointment under the patient appointment view tab (you may need to add this).
