It would be helpful when I change a medication the old dose and frequency should be included on the new Rx. I still get faxes for clarification from the pharmacy. For example, I recently changed a patient's Diamicron from 90 mg qam and 30 mg qpm. I had Accuro write to "stop duplicate Rx" but as it did not list the old dose was being stopped I still received a request for confirmation that the old dose was being changed. I receive a number of clarification faxes from the pharmacy if the Rx didn't just say to stop the old medication, but also lists the sig from the old Rx.
