Choose length of Custom Field

Allow users to choose length of custom fields! We have several that we use (on the advice of QHR, because we track data that does not otherwise fit into the system) and because each field takes up an entire line there is a lot of wasted space on the screen. Even small, med, large options would be helpful, but the ability to specify the number of characters would be even better!

Also if you we can customize how many lines, that would be great (similar to the global message box)

Ability to make notes in the patient demographic section Merged

The current "note" space is small and only shows one item at a time. I would like to see a larger field that could hold upwards of maybe 5 different comments that we need to see without switching to another tab.
