Display provider names when running a Home Clinic Enrollment Provider query for an office

When running a Home Clinic Enrollment Provider query to extract the list of enrolled patients to an office or provider(s) we don't have the ability to list the providers in the query. This would be especially beneficial if we run this type of query for an entire office rather than for each individual provider.
