Blank out the Created date between uploading different scanned documents Merged

We scan in documents to the EMR from many current and past patient visits in various locations. The fact that the Created date box keeps whatever the last date entered in it was after a document is uploaded has caused some trouble before, as staff don't always realize they must double check to ensure it is the correct date. It would be nice if it blanked out between each document uploaded.
