When viewing the audit log sorted by Date/Time, some actions end up out of order as there are no seconds to distinguish when it happened. Quite a few actions can happen in one minute. Example, the audit log shows a form was printed before it was added to the patient. Please update the audit log to have hh:mm:ss
There is a System Setting that needs to be enabled and then you will be able to see the seconds in your Audit Logs.
In the Accuro Menu > Users > Manage Security > System Settings > Security > Show Seconds in Audit Log Entries > Save Settings
