Add a component to forms to add a footer for our providers

We are looking at using a form as a reporting mechanism to our referrers instead of letters as you can add boxes, lines and all in all make the report look more aesthetically pleasing. Currently we can add our providers information such as prac ID, name and office however when you add speciality, it has character limitations and overall it would be nice to just add the already snazzy looking footer.
