Linking Mom/Baby charts for midwifery practices

For midwifery practices, once the baby is born, every time the client has an appointment with the midwife, the midwife has to chart on both the mom's chart and the baby chart. Would prefer a clickable link in the mom's chart that takes the user to the baby's chart and vice versa in order to quickly toggle between both patients while documenting. Ideally, it would be best if the two charts could be opened simultaneously and distinguished via tabs at the top above the status bar, so the user could click one tab and see the mom chart, and click the other tab right beside it to see the baby chart. If the client is prenatal, this side-by-side tab feature would disappear as the user would only be assessing the one patient/client at that time. For this to work, I believe the mom-baby need to be linked by the office once the client has given birth, and then either unlinked at the end of care, once the mom comes back into care for a future pregnancy (a task our office would be willing to do for this type of feature).

Also, would like the system to automatically convert to days/weeks postpartum once the client has delivered. Age of baby will automatically show up in the baby chart, but would like that same data to appear in the mom's chart.

Update, I meant we would like the status bar to convert to day/week postpartum once the client has given birth. I wasn't sure if that was clear above. Right now, our status bar shows gestational age. We would like that to automatically move to age of baby (or days/weeks postpartum) once baby has been born.
