Having MOH forms imported into Accuro

I find it is impossible to import PDF forms from MOH (death cert, MAiD physician aide forms, etc) into accuro for the form editor tool.

I did convert one of those (Physician aide C) forms from pdf to jpeg, but then each page saved as a separate jpeg file. I don't know if there is a way to merge pages into one document on Form editor.

If we could do this, it would be easier to fill out, and to keep a copy in patient's chart
