When printing or populating medical hx bands into to other documents or as an attachment (referring specifically to surgical/medical history, history of problems -ANY of the medical hx bands within really) -the start date, end date and any added notes must ALWAYS be included in any use of the information from these bands. This is vital information which is NOT being included currently. This is a problem. These notes/dates are really important. At the very least, give us as individual users a check box to turn the addition of this information on and off as needed. Thanks, Samantha
