Please make Sign-in option more visible for online booking

When booking an appointment many users are distracted by the create an account form in step 4. This is leading to confusion on the patient side, and issues with Providers linking the patient to the correct account. There is a sign in link in light blue, but many users see the cursor move the the name field and they look right over it. Please make it so the user has to choose to sign in or create an account.

    When they are booking online, they should have sign-up option beside sign-in so they don't have to go through 3 steps before to sign-up.
