'Date Unknown' Option for Immunizations

Often when new patients join a clinic, they have had vaccinations but may not have a schedule/schedule of information may not be available from their previous provider. Being able to select 'date unknown' or no date would be advantageous in these circumstances, so immunizations external to the clinic can be accurately documented.
