Separate tag for GLOBAL referring provider vs next appointment referring provider.

Currently the tag and is taking the referring provider from the next appointment. This means that if you update in demographics the referring provider it will not be used by those tags. It would be good to be able to have the option to use the Global Referring provider or the Appointment Referring provider when using tags as to decrease any confusion as to where the information is being pulled from (and also to be able to update global provider and have it properly reflected).
