[OK] button after referral generation must give option, " Add referral to cue?"

Necessary as the workflow mistake is obvious:

  1. Physician generates a letter.
  2. Physician edits letter in the generation window. 
  3. Physician chooses [OK] not realizing the letter was not added to the letter cue.
  4. Staff never learns of the existence of the letter. 
  5. Patient referral is delayed and safety compromised. 

An obvious error that certainly has happened and will continue to happen.
