Ability to cancel appointment that has a linked Note/Form (DOS) Merged

We send out notices for every appointment we book. Sometimes appointments become not needed and we need to cancel.

If an appointment has a linked form, there is no way to cancel the appointment unless we delete the form or unlink the DOS.

We should not have to delete or unlink the sheet in order to cancel an appointment.
