Auto-generate a form when creating an appointment

We would like to auto-generate 3 different forms when creating an initial appointment. Every time a new patient comes to our clinic, we generate the same 3 forms, so it would be helpful if this was done automatically.

    I love this idea! As a specialists office, it would be beneficial to be able to pick type of appt and change which forms are auto created as well.
