When creating forms that require a facility name, it would be useful to be able to pull the provider demographics office name into the form. Currently you can only pull the demographics address, phone and fax and the general Office name, but providers within the same clinic may have different "office names". What I have had to do to create this form is use a dropdown. There have been instances when the staff forget to select the correct office name.
