Our specialty clinic has clinicians who need to order labs/x-rays and medications using medical directives. The user name needs to appear as the implementer of the medical directive and indicate which provider is the prescriber who authorized the user to implement the medical directive. It is the authorizing prescriber that needs to be the one contacted for refills and to have their name on the lab requisition. The user didn't technically "prescribe" the medication or order the labs/x-rays so it needs to have the physician name appearing as the prescriber/authorizer so they are the ones contacted for refills and/or be the provider notified of lab test results and x-ray results. However, for legal liability reasons and proper documentation the user needs to also document that the order (medication/lab req/x-ray req) that they implemented the medical directive and why. See also my other idea submitted about the "authorized by" and "prescribed by" feature in prescription settings that don't print on the prescription so are actually useless fields. The authorizing provider (in our clinic it is a physician) needs to appear on the letter head of the prescription which can work by using the "authorized by" field. The user signature should then appear onthe prescription but with some way to document easily that the prescription should have the physician name on it and not the user. eg. Prescription"as per medical directive xyz" Implemented by "user name" under the authorization of "provider name". The "user" signature would be required for legal purposes but the Pharmacy that receives the prescription needs to easily see which "provider" (ie physician in our clinic) name should be appearing on the prescription. Same goes for lab reqs and x-ray reqs. Currently when we print them out, we can put the provider name on it and it will include the provider signature on the form but there is nowhere for the user to document "implemented by user according to Clinic medical directive xyz under the authorization of provider".
