Waitlist: Have "Decision Date" Auto-Populate to Generate a "Book-By" Date for a Surgical Request

When booking a Consult Request the Request Date auto-populates with todays date to generate a Book-By date off of the Priority. When booking a Surgical Request the Decision Date has to be manually entered to generate a Book-By Date off of the Priority. If a patient is being put on the Waitlist for surgery, a decision has already been made and should default to the date the request is created.
