Exporting documenst to Word or Excel in addition to PDF format

Currently we have the ability to export forms into PDF format. It would be able helpful to be able to directly export into the commonly used formats of Word or Excel documents. This will allow to edit this documents without the extra step of converting PDF files to Word or Excel thereby avoiding the extra steps and formatting problems.

This is most useful in OR booking forms that asks for extra dates such as date referral made, patient first seen and date of decision to operate. Most of these data is available in the wait list component of Accuro but they can not be exported to forms.
