Adding clinic specific reportable boxes on the INR Tracking form under the Lab quick buttons

Statistics on patients taking a blood thinner need to be tracked and reported on.  For example, if the patient is within target range and/or if they had a recent bleeding episode.  We would like to include this on the INR tracking sheet and not have to create another form.   The reportable boxes would need to be labelled by the users to collect the information that is required, as the reportable information is clinic specific.
