Allow multiple procedure codes to be added to appointment type defaults

We have patients who come in for multiple services, which coincide with the different appointment types we have, so it would be nice to have preset procedure codes that are attached to appointment type defaults. Example: when we book an appointment for a patient who will be getting shockwave and a physiotherapy treatment, these are two separate procedure codes that we would like to be attached to an appointment type that is different from just a shockwave or just a physio treatment. This means we set it up to automatically generate one bill for shockwave, but we have to manually enter the additional code. This leads to incorrect billing. Example 2: the physician usually uses two or more codes for complex consultations, but we can only choose one procedure code to attach the appointment, so the physician has to manually add in the additional codes. This takes additional time.
