Workflow: One Provider step! How to use Tasks to enhance health promotion, improve office efficiency and trigger Accuro billing and tracking modules. (Accuro Key Words: Billing / Labs / Documents / Interfaces / Tasks / EMR / CPP / Query Alerts .)

____________________________________________________________________________ Context: Medium sized group, Ontario, Capitation and Pay for Performance, Mammo, Pap, FOBT __________________________________________________________________________ EMR Gap and Clinical Need: Patient Question: "When is my next Pap? (mam, fobt, scope etc)" -------------------------------------------------------------------------------------------------------- EMR's are useful to find and record that a maneuver is done but to determine when the next maneuver is due one must know the current guidelines, the patients history and risk group, and the previous as well as most current result. ___________________________________________________________________________ Objective: Have Providers do one step , appropriate to their skill set, and then deploy Accuro features to their fullest. --------------------------------------------------------------------------------------------------------- Setup: 1)In Manage Tasks create the following Tasks for dropdown inclusion: Next Mammogram, Next Fobt, Next Pap, Next Colonoscopy, Next Wellness, Next Echo, Next Bone Density, Next Other, 2) Providers set the default task recipient to themself (Home / User Preference) Use: In Home > Inbaskets (Labs and Documents) when a Provider signs off on incoming results they hit F7 , choose the appropriate "Next" task, indicate the time interval and save. Example, Colonoscopy results come in and knowing the patient, FHx of ca colon, the scope report (n/l) and the guideline the Provider would set a Next Colonoscopy Task at 5 years. Similarly for Pap, Mammogram, FOBT etc the most knowledgeable person sets the "Next Pap", "Next Mammo" etc Task in place. Utility: What cascades then is all users can readily F7 any patient from any module (eg MOA from scheduling) and see when the patient is due for Pap, Mammo, etc. (Saves time ++, answers the question at hand with one button, and saves mis-application of guidlelines and patient risk by those who don't know the patient) Patient Question: When is my next pap? Answer = hit F7 Now, ...harness Accuro: Behind the scenes, eg once/month, Run Query Alerts and search for all "Next" Tasks, and use the ability to mass bill or apply tracking codes to the collective output. Example: We search for the Next Fobt (tracking code Q133 q30months) task that has NOT had a Q133 billed in the last 30months and is due in the future. This is fast and simple. The output is date sequenced (touch the button) and those maneuvers due in the next OHIP year (meaning they are current this year) get the code applied by highlight all, select Apply Bill and apply Q133. Consequence: F7 is a snapshot of Next Maneuvers due and dates available to all users. Billing modules (incl. PCB module in Ontario) are updated en masse behind the scenes. Accuro Tracking in Encounter window is updated en masse. In baskets start to remind providers of overdue Tasks In baskets can be used to find Tasks due ( eg 1w, 1m, etc) and delegate proactively to get the work done. Query Alerts can be used to create group wide lists of upcoming Tasks due so this work can be delegated proactively. Query Alerts can be used to find those with No Next Tasks = those missed screening - Query Alerts can be used to aggregate those Overdue for a Next Tasks Enhancement: When generating a Next task i jot down in notes info impt to speed the next apllication of the task ... eg in Next Mammogram I will note FHx +, this makes it quick next year to set the Next Mamogram at one yr without chart review. ____________________________________________________________________________ Ultimately your practice has Providers use their unique knowledge of the patient, the guidelines and results to date, to do one step and harness multiple features of the software to generate accurate and current data that serves all users and patients. Hope it helps, share with collegaues, and tweak for your own province / practice needs. stephen
