Allow for Multiple Modifiers in Billing Macros

In Nova Scotia, multiple modifiers are frequently required for a single service code. For example, 03.03 requires 2 modifiers for patients over the age of 65. Right now, only one modifier can be added to a billing macro which means that each code still needs to be edited when the macro is applied.
