Create More options for developing new custom demographic indicators

At present, we can only create new demographic indicators using Drop-down or Text fields. However, with the expansion of emerging services that we will offer to patients, we require additional field types, such as Date, to better capture and report relevant information. It's important to note that these patients may not be formally enrolled in our system, meaning an Enrollment Date would not apply.
