you can add a popup alert and assign it to specific roles within your clinic - Next to Global Message, click the green plus sign and a new box will open below the global message - use the dropdown to assign the alert to a role and only those assigned that role will get the popup when opening the patient's chart.
Unfortunately you cant assign it to a specific physician - you could assign it to all physicians.
