Providers no longer at clinic are marked "Historical Provider" and opens up all chart items and no longer needs permissions to be managed

Providers who have moved to another clinic or have passed on should be able to be marked as a "Historical Provider".

This is when there is no need for them to have locked chart items or any permissions set in manage security to limit users to their chart items. All chart items they have created would now be open and available for all users and providers.

This helps clear up the manage security > templates > permissions template management. We do not need to see providers that are not in our clinic and do not need permissions granted. It clutters up the space here where we need to manage active providers in our clinic.
