Add "Alias" as a Query Field in Forms

It would be useful to be able to use the "Alias" field as a query in the form editor. It is sometimes necessary to have names available on forms that are different than a patient's legal name. If this information is already being gathered then we should be able to use it as another identifier for patients, especially when government forms/requisitions are already including this as a field on their forms.
