PUBLISHED FORM - Organization

Digging through these forms is way too time consuming and very hard. Forms Should have columns for 1. Province - ie Ontario 2. Institution/facility/clinic/ - ie Royal Victoria Hospital 3. Name of the Form - ei. CT requistion 4. Date of the Form - as is already there. 4. Specialty - don't find necessary. The form naming convention should be set up as well facility name of form. Which would make it easier to find in the future. Also would be nice if you click on the form name that it opens in a preview screen instead of clicking on preview
